Care for those close to your heart

Valley Health Partners' primary and family care goes beyond appointments and prescriptions—it's about neighbors caring for neighbors.

Valley Health Partners (VHP) delivers compassionate, accessible, and affordable care for all ages—from infants to older adults—through more than 20 locations across the Lehigh Valley and Poconos. Their primary and family care goes beyond appointments and prescriptions—it’s about neighbors caring for neighbors.

More than 50% of colleagues are bilingual, and many grew up in or live within the communities they serve. That deep connection fuels a shared understanding of patients’ lived experiences and a commitment to meeting people where they are. Whether it’s providing preventive screenings or managing chronic conditions, the VHP team is rooted in building lasting, trust-based relationships across every stage of life.

Providers manage wellness visits, routine screenings, vaccinations, acute and chronic illness, mental health concerns, and disease prevention. Their integrated care model makes it easy for patients to connect with specialists, screenings, education programs, and other services within VHP or through trusted community partners to support whole-person health and well-being.

Combining clinical excellence, compassion, and cultural awareness, VHP delivers family-centered, patient-first care from people who truly understand the communities they serve.

On October 11, VHP will hold its first annual Sole Mission Walk at Cedar Crest College from 9AM–12PM. This fundraising event benefits the VHP Street Medicine Program, a vital team that delivers essential primary and urgent care to individuals experiencing homelessness throughout the Lehigh Valley.

This family and dog-friendly event features kids’ games, live entertainment, food trucks, and a community resource fair. Join VHP in walking together to raise awareness and critical funds for our neighbors in need. Learn more and register at givebutter.com/SoleMissionWalk.
