At Valley Health Partners, health is shaped by so much more than what happens in the exam room. That’s why the organization works alongside more than 120 community partners to expand opportunities for health and build a community where everyone has the chance to thrive.

Its clinical care and community partnerships are guided by what Valley Health Partners hears directly from patients through social drivers of health screenings. For both adults and children, the most common challenges include food insecurity, transportation, housing instability, and financial strain. Together with its partners, Valley Health Partners is helping remove these barriers so more people can access the resources they need to live healthier lives.

One example is Valley Health Partners’ partnership with Kellyn, bringing the Eat Real Food Mobile Market to its South Side Easton location. By offering fresh produce and nutritious ready-to-enjoy meals—and accepting WIC, FMNP, EBT, cash, and credit—the partnership is making healthy food more accessible for the community.
